JOB PURPOSE You will be part of our community outreach team, also known as the Community Resource, Engagement and Support Team (CREST), focusing on raising public awareness of mental health conditions and dementia, early identification of at-risk individuals, providing emotional support to individuals and their caregivers, and linking individuals to relevant health and social care services where necessary. You will be placed within the Community Mental Health Department, under the Mental Health & Counselling Services Group. KEY RESPONSIBILITIES Case Management Provide case management services across assessment, systems work, case conferences, and closure, and maintain proper and up-to-date case documentation Attend to enquiries and provide information and referral services Conduct basic emotional support for clients in need Clinical Group Work Plan and facilitate group interventions, including case conferences with significant others and relevant partners Evaluate and review group interventions to enhance programme quality and effectiveness Outreach & Programme Planning Plan, coordinate, execute, and evaluate ongoing programmes and services, including outreach, on-site and online public education Conduct analysis, and develop and implement mental health programmes and services to address community needs and trends Engage key community stakeholders to coordinate and implement mental health programmes Coordinate with other professionals and mobilise community resources for service delivery Evaluation & Reporting Monitor programme outcomes and evaluate impact through qualitative and quantitative data analysis Utilise findings to make data-driven recommendations for programme enhancements and future initiatives Maintain programme data, records, and documentation, and prepare and submit periodic reports for key stakeholders Supervision & Volunteer Management Provide supervision to junior caseworkers, volunteers, and interns Professional Practice Demonstrate professional
