JOB PURPOSE You will be part of our community outreach team, also known as the Community Resource, Engagement and Support Team (CREST), focusing on raising public awareness of mental health conditions and dementia, early identification of at-risk individuals, providing emotional support to individuals and their caregivers, and linking individuals to relevant health and social care services where necessary. You will be placed within the Community Mental Health Department, under the Mental Health & Counselling Services Group. KEY RESPONSIBILITIES Clinical Leadership & Supervision Provide clinical leadership for the implementation of the CREST programme according to the Tiered Care Model, and ensure service requirements are met Oversee clinical case supervision and staff development of social workers Monitor supervisees' compliance with professional processes (such as the use of tools and templates, and adherence to practice policies) Conduct coaching and performance management reviews with staff Induct new hires in the centre through individual supervision during their probation period Case Management & Direct Practice Provide case management services across assessment, case management, systems work, case conferences, and closure, and maintain proper and up-to-date case documentation Attend to enquiries and provide information and referral services Programme & Operations Management Establish and operationalise programme-level SOPs and clinical workflows Plan, coordinate, execute, and evaluate ongoing programmes and services, including outreach, on-site, and online public education Oversee the tracking of programme data and prepare submissions of reports to the programme funder Support the Service Head and Principal Social Worker in the implementation of the centre's operational and practice protocols, processes, and policies Stakeholder Engagement & Partnerships Engage key community stakeholders to coordinate and implement mental health programmes Network with communi
