Conduct needs and risk assessment in accordance with the service framework. Implement and follow up on case plans and interventions. Work with community stakeholders to address systemic barriers to reunification. Provide regular updates and reports to the interdisciplinary team to facilitate decision making on case plans for the clients. What We’re Looking For Degree or Masters in Social Work with minimum 2 years (or minimum 4 years for Senior Social Worker position) of related work experience in children, youth or family work. Willing to work after office hours as this is a 24/7 based service and flexibility in work arrangement is expected.
