You will play a critical role in bridging operations, clinical coordination and patient engagement—ensuring care delivery is efficient, responsive and compassionate. Key Responsibilities Enable Seamless Care Operations Support Ops Team Leads and work closely with clinicians, nurses and allied health professionals to ensure smooth day-to-day programme operations Coordinate patient journeys from enrolment to discharge, ensuring efficiency and accuracy at every touchpoint Manage scheduling, appointments and care transitions to support continuity of care Deliver an Excellent Patient Experience Conduct service introductions and facilitate patient onboarding and consent for WH@Home enrolment Provide financial counselling support and assist with subsidy applications and documentation Maintain clear, accurate and timely documentation of patient interactions in systems such as EPIC Handle patient feedback, enquiries and service recovery with professionalism Support Clinical Operations Prepare patient case files for home visits and teleconsultations Assist with patient discharge processes and required documentation Adhere to infection prevention protocols wh
