Assist in managing client inquiries and coordinating requested services. Participate in case discussions and multidisciplinary care planning. Assist in coordinating referrals with internal teams and external community partners. Participate in home visits with care teams to observe and support client assessment, care delivery, and follow-up. Engage with clients and caregivers during home visits and telephone follow-ups to understand needs and challenges. Support caregiver education by reinforcing care techniques, resources, and available support services. Collaborate with social and healthcare partners to recommend appropriate car
