ROLES & RESPONSIBILITIES Programme Planning & Execution Programs Management: Create and manage programmes that fulfils the requirements of Active Ageing in Singapore. Daily Events Management: Plan and run daily activities like workshops and social events that help seniors stay healthy and active. Review Progress: Use attendance records and feedback from seniors to see how well programmes are working and how to improve them. Community Outreach & Engagement Find & Connect: Identify vulnerable seniors in the neighbour and support them through home visits, material support and referral of services. Build Trust: Develop good relationships with seniors and their next of kins and caregivers to understand and support them. Collaboration with Community Partners: Team up with local groups and stakeholders to promote the centre’s services and build strong partnerships to support the senior community. Volunteer & Community Partner Management Lead Volunteers: Recruit, train, and supervise volunteers to help with centre activities and reaching out to the community. Maintain Partnerships: Keep in touch with sponsors and community partners to help grow the programmes we offer. Care Coordination: Work with healthcare partners and other social service agencies to make sure seniors get the help they need through clear referral paths. Supporting Seniors Buddying and Befriending Programmes: Support isolated seniors and matching them with the right volunteers. Information Services: Handle questions from people who call or walk in looking for information about senior care. Service Referrals: Directly help seniors find the right social or healthcare services based on their specific needs. Case Management: Keep an eye on more complex cases to make sure these seniors are getting regular support and follow-ups. Admin & Operations Keep Records: Maintain clear and accurate records in AAC recording including attendance, member files, and programme reports. Centre Operations: Help the supervisor
