Job Summary
The Population Health Virtual Pharmacy Team utilizes population health management strategies and virtual communication to care for patients across the Advocate Health Midwest Region, focusing on chronic disease management and patient self-management.
The Population Health Clinical Pharmacist – Heart Failure will focus on post-discharge follow-up for heart failure patients via telephone and/or video visits, joining a team of pharmacists and technicians.
This is a remote position requiring candidates to live in IL or WI and obtain dual licensure within 90 days, with hours from 8 am to 5 pm, Monday through Friday.
